Patient Information & Consent Form (Updated August 2021)
All persons before their first visit to the clinic since before lockdown were introduced on 23 March 2020, will be sent a link to a specific page on our website which contains an “Information & Consent Form – Risk of Coronavirus (COVID-19) Transmission at The Clinic at Borde Hill”. All persons must agree to all points and electronically submit their form before attending for their first appointment since lockdown. On return visits, they will verbally be asked if they still agree. Copies of this form will also be available in the clinic if needed.
